Aseptic Cystitis is Cystitis that is not caused by bacteria, but by other factors. The symptoms are identical to those of bacterial Cystitis (urgency, frequency, pain when urinating, bladder heaviness), but nitrites are always absent in the urine. Urine culture is negative. Many leukocytes and blood may be present. If there are no leukocytes, the cause of the pain is not urological, but must be sought outside the urinary tract.

Due to medical protocols aimed at reducing healthcare costs antibiotics are often prescribed without a urine culture to confirm the actual presence of bacteria and, unfortunately, too often despite a negative urine culture.

In reality, Cystitis can be caused by dozens of non-bacterial factors that explain why antibiotics are ineffective:

  • food or drink intolerances
  • sexual intercourse
  • stones and gravel
  • autoimmunity
  • excessively acidic pH
  • bladder endometriosis
  • interstitial Cystitis

These factors cause irritation and inflammation similar to that caused by a bacterial attack. What causes pain is the inflammation that the bladder produces to defend itself against the attack. Whether caused by bacteria, acidity or other irritants, the resulting inflammation is the same and therefore the symptoms will also be similar. In this case, leukocytes will be found in the urine test strip.

What needs to be combated, therefore, is not the bacteria (with antibiotics), but the causes that generate inflammation. We must therefore:

  • avoid urinary acidity through oral alkalising agents and a healthy diet
  • strengthen the bladder walls with GAG-based supplements (Purorepair GAG Plus, for example)
  • take anti-inflammatory drugs, preferably natural ones
  • drink soothing herbal teas (mallow, couch grass and horsetail)
  • prevent bacterial infections (with D-mannose)

If, on the other hand, there are no leukocytes in the urine, it is possible that the cause of the symptoms is due to non-urological conditions and disorders (much more common than is believed) that cause symptoms similar to those of Cystitis:

  • Vulvodynia
  • pelvic muscle contracture
  • pelvic neuropathy

It is therefore essential to recognise and treat these conditions in order to eliminate the underlying symptoms.

A separate discussion is needed for the very few false negatives, i.e. those cases in which the urine culture does not show the presence of bacteria, even though they are actually present.
In this case, the same treatment as for bacterial Cystitis should be followed.
