Genital infections are always the result of a vaginal imbalance involving three altered factors: an increase in pH, a decrease in lactobacilli, and weakening of the genital mucosa.

Instead of fighting pathogens with antibiotic and antifungal therapies—which have definite side effects—it would be far more effective and harmless to work on the main causes by restoring vaginal lactobacilli levels, lowering the pH, and strengthening the mucous membranes. Oral treatment:

The oral therapy aims to regulate intestinal function and reduce the excess of pathogenic bacteria or Candida in the digestive system.

This goal can be achieved by introducing beneficial bacterial flora (Saccharomyces boulardii and Lactobacillus acidophilus), lactoferrin (which has antibacterial, antifungal, and immunostimulating properties), acetylcysteine (which breaks down the biofilm where bacteria and Candida hide, depriving them of an important defense), and bromelain (to reduce inflammation and soothe pain).

  • first and second week of treatment:
    Purodefend Urto (1 capsule in the morning and 1 capsule in the evening) + 1 Purocol Flor at lunch with food
  • maintenance phase: after the acute phase, to prevent relapses, take Purodefend Urto every morning during the week before your period. About 3 days before menstruation, increase the dose to 2 capsules (one in the morning and one in the evening) and continue for the first 2 days of your period. Local therapy:

Local treatment will help restore the correct vaginal pH, rebuild the beneficial bacterial flor, fight existing pathogens and strengthen the vaginal mucosa and defenses.

  • On the first, second, fourth, and fifth days: use Purofem DÖDERLEIN or Calagin vaginal (both containing lactoferrin and probiotics to combat Candida, E. coli and other pathogenic bacteria while simultaneously introducing lactobacilli)
  • Third day: yogurt (read about our experiences with yogurt in the forum)
  • Seventh day: D-mannose-based vaginal wash (to remove residues from previous applications and to deliver mannose into the vagina)
  • Every morning, apply Puronerv Oleogel to strengthen the vulvar mucosa, protect it from pathogens and reduce inflammation

Start again from the beginning and repeat the same schedule for another 7 days.

Then reassess the situation based on symptoms or a vaginal swab. The need to start or repeat the Miriam protocol can also be evaluated by vaginal pH: if lactobacilli are insufficient, they won’t adequately acidify the vaginal environment, resulting in a high pH. A pH between 3.5 and 4.5 indicates an optimal amount of lactobacilli. A pH above 5 suggests the need to (re)start the protocol. This parameter can be easily checked at home using vaginal pH test strips, tests for detecting vaginal infections, or through a specific analysis of the vaginal microbiota.

After the acute phase, until all pelvic balances (vaginal, urinary, muscular, neurological) are restored, it is advisable to apply Purofem alternated with yogurt during the week before menstruation (the phase when symptoms tend to worsen), then stop at the start of menstruation. After menstruation, continue applying yogurt and Purofem on alternate days for one week, as the alkaline nature of menstrual blood raises the pH, hindering the survival of lactobacilli.

After the acute phase, it will also be important to combine a local restructuring and re-epithelializing treatment based on soothing, emollient, and antioxidant substances. These help repair the cellular damage caused to the mucosa by the infection, preventing bacteria from penetrating the more fragile deeper layers. These antioxidants are found in plants (polyphenols and bioflavonoids), vitamins (vitamin C, vitamin E, carotenoids such as quercetin and lycopene), and micronutrients (selenium, zinc, copper, glutathione, coenzyme Q10, melatonin, uric acid, and hyaluronic acid).

Behavioral advice:

  • eliminate synthetic pads and switch exclusively to disposable cotton pads, washable pads or a menstrual cup
  • avoid using panty liners on non-mestrual days, as they prevent the skin from breathing
  • wear only white cotton underwear, which allows the skin to breathe and provides oxygen that inhibites anaerobic bacteria
  • stop using all intimate cleansers to avoid removing the natural Döderlein flora
  • avoid using bicarbonate or tea tree oil in the bidet, as these have proven too harsh on the mucous membranes

  • sleep without underwear
